CHS225 Implement a treatment plan

Overview

This standard relates to the implementation an individual’s treatment plan. This may be undertaken in a wide variety of settings and healthcare individuals. Users of this standard will need to ensure that practice reflects up to date information and policies. Version No 1

Knowledge and Understanding

You will need to know and understand:

  1. your own level of competence, authority and specialist knowledge base
  2. the range of stakeholders required in implementation of a treatment plan and their information needs, roles, responsibilities and capabilities
  3. how to communicate effectively in the appropriate medium to meet any recipients’ needs and preferences
  4. the  importance of checking the individual’ identity, agreement and valid consent/authorisation for the treatment plan or intervention and how to do so
  5. the range of health and safety measures, infection prevention and control and the relevant personal protective equipment, their importance and application within your work practice
  6. the importance of explaining the treatment plan or intervention and as appropriate, the frequency of interventions within a treatment plan,  with the individual and/or relevant others
  7. the importance of ethical issues surrounding the provision of treatment plans and how to implement these
  8. the range of relevant protocols and procedures and their interpretation for any required treatment interventions within the treatment plan
  9. the relevant underpinning principles, purpose and application of the relevant science, technology or engineering within your scope of practice applicable to the treatment plan or intervention
  10. the range of issues and contra-indications that required consideration for the appropriate treatment plan or intervention for individuals and their relevant others
  11. the range and types of support needed by individuals and how to identify them
  12. the factors affecting the suitability and effectiveness of appropriate treatment plans or interventions for the individual’s condition
  13. the range and types of problems which may occur with treatment plans and how to prevent these
  14. the importance of working collaboratively with multi-disciplinary teams and how  to seek advice and support from other colleagues
  15. the range and types of equipment, medical devices and products relevant to the treatment plan within your speciality and relevant to the individual’s needs
  16. the measurement  parameters required for interventions involved in the treatment plan
  17. the sources of reliable information and their importance for the individual or relevant others regarding their treatment plan or intervention and how to access them
  18. the importance of recording procedures and documentation relating to treatment plans
  19. current legislation, national guidelines, organisational  policies and protocols, clinical and information governance within your work practice
  20.  
  21. the policies and guidance that clarify your scope of practice, accountabilities and the working relationship between yourself and others

Performance Criteria

You must be able to do the following:

  1. work within your level of competence, responsibility and accountability
  2.  
  3. check the individuals identity and check valid consent/authorisation to the  treatment plan or intervention
  4. apply appropriate health and safety measures, infection prevention and control and personal protective equipment within your work practice
  5. review relevant data and information to ensure the appropriateness of the proposed treatment plan or intervention to the individual’s current status and likely future needs
  6. select the appropriate treatment plan in accordance with local protocols, procedures and identify any special requirements to meet the individual’s  needs
  7. clearly explain the proposed treatment plan with the individual and /or relevant others
  8. where appropriate, select  suitable equipment, medical device or product and any other requirements for the treatment plan or intervention
  9. confirm with the individual and/or relevant others their agreement and authorisation for the selected treatment plan or intervention
  10. provide information  for any required skills or activities to enable individuals to follow the treatment plan or recuperate as appropriate in their own environment
  11. provide other stakeholders,  who are to be involved in monitoring the treatment plan with sufficient and timely information in accordance with organisational policy and protocols
  12. record all  findings, discussion and agreements and report accurately, completely and legibly using appropriate documentation processes and in accordance with confidentiality requirements
  13. issue a report to stakeholders at sufficient level of detail with any recommendations, findings and intended outcomes of the treatment plan
  14. confirm that individual and relevant others have relevant documentation and understand what further action will be taken
  15. store and maintain full, accurate and legible records of information collected in line with current legislation, guidelines and local policies and procedures
