CHS173 Develop care pathways for patient management

Overview

This standard relates to developing and planning of long term; multidisciplinary care pathways for specific patients and carers. Users of this standard will need to ensure that practice reflects up to date information and policies. Version No 1

Knowledge and Understanding

You will need to know and understand:

  1. Scope, purpose and limitations of care pathways
  2. The range of care options available and how to access relevant services
  3. The advantages and disadvantages of each care option
  4. How to match care options to patients’ needs
  5. Methods and measures of evaluating progress and outcome of care pathways
  6. The implications of care options for patients
  7. The implications for different patient capabilities on selection of care options
  8. Local availability, cost and access to selected care options
  9. Procedures for implementing care options
  10. Recording procedures and documentation relating to care plans
  11. Communication channels with other services, professionals and agencies who may be involved in delivering the care plan
  12. The purpose of clinical review and audit
  13. How to review and audit care pathways in practice

Performance Criteria

You must be able to do the following:

  1. plan pathways for patients and carers based on discussion of options, priorities, concerns and preferences of stakeholders
  2. agree, review and evaluate methods and arrangements
  3. aim to optimise patient care within constraints of patient preferences, cost and availability of options
  4. ensure that the pathway allows for patient and carers to have clear and realistic expectations about the results of planned outcomes
  5. agree and gain consent for next actions
  6. provide timely information to other professionals or agencies who will be involved in implementing the care plan
  7. review and audit care pathways to meet emerging needs and contingencies
